The letter of Mr. Kevin Engle left me with the afterthought, "Speed" (Engle: Accidents do happen, and new standards are needed). The near miss was because of less speed.

There seems to be blame all around, so to blame one and not the other is the problem for me.

A very good example. Who would deem it safe to travel on water at any speed when visability is limited?
